Health & Welfare Benefits - National Elevator Industry Benefits Program - IUEC

Comprehensive Medical Benefits - covers doctors visits, lab tests, surgery, hospital stays, physical exams and wellness program.  Annual Deductible per person $300.00, per family $600.  Wellness programs and physical exams are not subject to deductibles, it is 100% covered.  No deductible is network.

Chiropractic Services - Limited to 28 visits per year - medically necessary.

Convalescent Facility - Benefits are payable for a maximum of 70 days per confinement.

Hearing Loss - Coverage to $1,200.00 for hearing aids in each ear every 36 months for adults, 12 months for children.

Prescription Drug Benefits - For all plan participants, there will be a retail pharmacy Co-payment of %20 of the prescription's total cost with minimum Co-payments of $5.00 generic - $15.00 preferred brands - %25.00 non-preferred brands and a maximum Co-payment of $40.00 for up to a 30 day supply and there will be a mail order Co-payment of $10.00 generic - $30.00 preferred bran - $50.00 non-preferred brand for up to a 90 day supply.

Dental Benefits - $1,500 per dependant annual benefit.  Annual deductibles $50.00 per person or $100.00 per family.  Annual cleaning's and check-ups are not subject to deductibles, and if you or your dependents require Orthodontic (braces) each will receive a $2,500.00 benefit exclusive of the $1,500.00 annual dental benefit.

Vision Benefits - Annual deductible $0, provides for annual exams, and one pair of glasses per dependent and a second pair of glasses for work for each member (Lenscrafters).  LASIC Surgery to $2,400.00 both eyes.

Life Insurance Benefit - $40,000.00 plus an additional $40,000.00 accidental death and dismemberment benefit payable if you lost your sight, hearing or limbs in a covered accident.

Mental Health, Alcoholism & substance Abuse Benefits - Mental health maximum benefit $50,000.00 lifetime benefit.  Alcoholism and substance abuse treatment - 60 day inpatient maximum lifetime treatment.

Weekly Income Benefits - $325.00 weekly for 26 weeks.  Weekly income benefits are paid to you if you become totally disabled (unable to work) because of an injury, accident, illness or pregnancy.  this covers an injury at home and non work related injury.

Extended Benefits - 6 months covered medical insurance for your family if injured on or off the job at no cost.

Benefit Extension - When you are out of work because of a workman's comp injury, or non-work related illness or injury, our benefits program extends to members 26 weeks of full family coverage.  This is important because when you are on workman's compensation insurance your family does not have insurance coverage.

Full Dependent Coverage - Eligibility - Legally married souse.  All unmarried natural and adopted children dependant on you for maintenance and support that are under age 19.  Dependents that are age 19, but under age 25 and are full time students at an educational institution.

Self Insured - The National Elevator Industry Benefits Plan is an insurance carrier for 25,000 elevator constructors and their dependents and our program does not change at the whim of any elevator company owner.  The stability of over $3.8 billion in assets insures you and your family will always have coverage.

This coverage costs each member only 3 1/2 cents per hour.
